Every applicant on a plain paper typed or clearly prints the following information:

  1. Your full name – last, first and middle, Underline the last name.
  2. Your date and place of birth like village, town, District, province and country.
  3. Your name, date and place of birth of your spouse and children, lf any (not ‘your parents).
  4. Your mailing address and telephone number.
  5. Your country of birth.

PLEASE NOTE:  1. There is no fee to be registered, nor do you need any middleman to be paid to mail-in your application, DO NOT PAY PER- SONS OFFERING YOU THEIR SERVICES TO DO YOUR REGISTRATION APPLICATION FOR MONEY RANGING FROM $50 to $200.

  1. There is absolutely no advantage to mailing early or mailing from a particular locale. Every application received during June 1, 94 and June 30, 1994 will have an equal random chance of being selected.

If you do that, your application will not be accepted.
